| You must obtain a valid University issued badge. Card reader access cannot be granted otherwise. Information on obtaining your badge can be found here: Aggie Card Office |

| Download a copy of the Badge Access Request Form by clicking here (PDF format), OR you can request to have a paper copy mailed to you (via campus mail) by sending an email message to itim@tamu.edu. You will need to fill out the form completely and then have your supervisor sign the form. |

| If you are requesting access to high security areas (as indicated in the Badge Access Request form), you will need to download a copy of the terms of access form. Click here (PDF format) to access the form or you can request to have a paper copy mailed to you (via campus mail) by sending an email message to itim@tamu.edu. You will need to read it thoroughly and then sign and date the form. |
| Step 4 |

| Put both forms into a campus mail envelope and mail them to CIS Information Technology Issues Management (ITIM) at Mail Stop 3142. ITIM will then verify your request with your supervisor and any other required parties. |
| Step 5 |

| You will be contacted by CIS regarding your approval status and access privileges. It is your responsibility that if your badge is lost or stolen you report the incident with the Aggie Card Office as soon as possible. |
